Application Forms
To enter the WorkSafe Awards, please select from the awards categories below to access the relevant application forms.
To simplify the application process you can save your incomplete form and return to it later. Just fill in your name and email address under the section called “Nominators Contact Information”, then click “Save and return later”. You will then receive an email with a link to return to your application form/s.
Every time you save your application form/s you will receive an email with a new link to your application form, so you can resume filling out your application.
To put together the strongest entry possible, view the online entry guidelines or call the WorkSafe Awards Coordinator on 03 8663 5033.
When you have completed your application form, click “submit” and you will receive an email to confirm your entry. You can choose to print your completed entry before or after you submit your form.
